7. Current HIV CEU
8. Domestic Violence
9. Alzheimer CEU 1
10. For all C.N.A and HHA an additional 12 Hours of In-Services is needed each year e.g. OSHA, Residents Rights etc. (NO Assisting with Medication) 1
11. Current Physical within the last six (6) months signed by a doctor which states you are free of any kind of communicable diseases including Tuberculosis.
12. Copy of Car Insurance
13. Auto Policy
14. Level 2 background check.
